Many people, many voices, one goal

NWMPHN receives annual funding from the Australian and Victorian governments.

Its goal is to keep people well, particularly those with chronic health conditions and mental illness, and reduce avoidable hospital presentations through the delivery of effective primary care.

At its heart, this is a massively collaborative task. The organisation works closely with general practices, mental health service providers, allied health workers and communities right across the 13 local government areas it covers.

To ensure communication and feedback are sustained, we are bolstered by the insight and expertise of our expert advisory groups, clinical and community councils, and members of our People Bank.

We are also grateful for advice provided by health professionals and consumers through our multiple survey and feedback channels.

With this wealth of knowledge and goodwill, we are better able to focus on meeting the needs of at-risk members of the community. Above all, we are driven by a vision for a primary care system that is person-centred, comprehensive, accessible, high quality and culturally safe.
