Building pathways: supporting primary health providers

NWMPHN’s planning and actions align closely with the Australian Government’s commitment to improving primary health care for all Australians through strengthening Medicare.

The core of this work lies in commissioning new services and supporting businesses to transition to new ways of working, ensuring that people encounter a system that is efficiently interconnected and has the capacity to meet demand.

Hand-in-hand with the sector, we are striving to modernise and increase access to primary care, encourage multidisciplinary teams, support digital and clinical referral pathways, and foster cultural change.

Our goal is to be a practical partner in supporting our health workforce to deliver safe, accessible, high-quality continuity of care.
